The key to understanding this is to keep questioning "why" a particular operator error was committed. When you get to these root causes, then, as managers, you can start working on eliminating those causes. So, with all that said, after viewing the American Airlines Flight 587 video, please discuss not only what you believe the root causes of the accident to be, but what could be done by all the players involved to try to eliminate these causes.
